Telehealth Informed Consent

Please read this agreement carefully. By completing your SofiaRX intake and checking the acknowledgment box, you are confirming that you have read, understood, and agree to the terms of this Telehealth Informed Consent Agreement ("Agreement").

1. What Is Telehealth?

Telehealth is the delivery of healthcare services using electronic communications between a licensed healthcare provider and a patient who are not in the same physical location. At SofiaRX, telehealth services are delivered asynchronously (through a written intake questionnaire) and, for certain plans, through video consultations.

2. The Services You Are Consenting To

By accepting this Agreement, you are consenting to receive:

• Clinical evaluation: Review of your intake, health history, and biometrics to assess eligibility for treatment.
• Prescription issuance: If clinically appropriate, prescriptions for compounded GLP-1 or peptide protocols.
• Ongoing oversight: Follow-up check-ins and medication adjustments.
• Laboratory services: Ordering and review of at-home lab panels.
• Sofia AI coaching: Support delivered by our AI health coach.

5. Risks and Limitations

• Technology failures: Sessions may be interrupted due to connectivity issues.
• Information limitations: A clinician cannot physically examine you or observe vital signs in real-time.
• No prescription guaranteed: Clinical eligibility is determined solely by the reviewing clinician.
• Geographic limitations: Services are only available where SofiaRX's prescribing network operates.

6. Compounded Medications Disclosure

You understand and acknowledge:

• Compounded medications are not FDA-approved.
• Compounded peptides are often considered investigational compounds and their long-term safety has not been established in large-scale trials.
• Certain medications require self-administration by subcutaneous injection. You agree to follow all instructions provided.

8. Sofia AI Health Coach

Sofia is an AI tool and not a licensed healthcare provider. Interactions with Sofia do not constitute a patient-provider relationship. Sofia's responses are informational and general in nature.

9. Your Rights

You have the right to:

• Withhold or withdraw consent at any time.
• Access your health records.
• Know the name and credentials of your reviewing clinician.
• Ask questions about your evaluation or prescription through secure messaging.

13. Acknowledgment and Consent

Your electronic acknowledgment during the intake process serves as your signature and is legally binding. You confirm that you have read this agreement in full and understand the nature of telehealth services provided by SofiaRX.
